We are currently seeking an experienced and compassionateRegistered Nurse to join our growing team and support the delivery ofhigh-quality, person-centred care within a nurse-led community careenvironment.

This is an exciting opportunity for a Registered Nurse withexperience in TDDI, complex care and clinical support, who is confident workingalongside care teams, individuals and their families to ensure safe, effectiveand clinically appropriate care within an office and community environment.

About the Role

As a Registered Nurse, you will play an important role in workingas part of a supportive and professional team, you will use your clinicalknowledge and expertise to ensure complex care is delivered safely, effectivelyand in accordance with care plans, clinical guidance and Bradcare's qualitystandards.

You will also work closely with service users, families,care staff and other healthcare professionals to promote independence, dignity,choice and quality of life for our service users.

Hours

This is a part-time position, working one day per week.

Exact hours and working arrangements are open for discussionand will be agreed with the successful candidate.

Key Responsibilities

• Providingprofessional nursing expertise and clinical oversight for service userswith complex healthcare needs within a community and nurse-led careenvironment.
• Supportingthe assessment, planning, delivery and review of complex care packages
• Usingyour knowledge and experience of TDDI and complex care to support safe andeffective care delivery in the community
• Supportingthe implementation and review of care plans, risk assessments and clinicaldocumentation and maintaining accurate, timely and confidential clinicalrecords in accordance with GDPR and professional requirements
• Supportingstaff with clinical procedures in accordance with training, competency andorganisational policies and ensuring care is delivered in accordance withrelevant legislation, professional standards and Bradcare policies and supportingsafeguarding processes and reporting concerns appropriately
• Monitoringservice users' health and wellbeing and escalating concerns appropriately
• Workingcollaboratively with GPs, OT's, community nurses, therapists, hospitalsand other healthcare professionals

About You

For this role we require the below:

• Afull UK driving licence and access to a reliable vehicle is essential
• Aqualified Nurse currently registered with the Nursing andMidwifery Council (NMC)
• Rightto work in the UK
• Experienceor knowledge of TDDI
• Excellentcommunication and organisational skills and able to work collaborativelywith individuals, families and wider healthcare professionals
• Confidentworking independently within community settings
• Ableto provide clinical leadership, guidance and support to care staff
• Passionateabout maintaining high standards of care
• Organised,professional and able to maintain accurate clinical documentation

Previous experience within community care, complex care ornurse-led care services would be highly desirable.
