What you will do:
- To provide safe, effective, person centred care to all our clients by following local and government policies and procedures.
- To act in accordance with the NMC to provide Nursing/complex care to clients at home.
- To always maintain safety for you as the worker, your colleagues and the individual being cared for.
- To support in the provision of evidence-based practice by sharing information provided by the clinical team to your colleagues.
- To work with as part of a multi-disciplinary team to enhance service user's satisfaction.
- To assess the service users' needs each visit and ensure that the care being provided is still appropriate for the client's needs.
- To ensure that all safeguarding issues are escalated as per policy.
- To contribute to a regional or local on call rota and cover for colleagues in the event of an emergency.
Your skills, knowledge and experience:
- Valid NMC Nursing PIN
- Ability to prioritise clinical tasks
- Experience with adults or children with complex needs
- A minimum of 12 months experience in a care or hospital setting
- A caring, patient, and positive attitude
- Willingness to be part of a family-centred team
- Demonstratable commitment to providing high quality, safe and effective care
- Have an understanding of safeguarding
- Excellent communication skills and be able to fit into a family setting
- Have the ability to recognise deterioration in both psychological and physical health
- Car driver with access to your own vehicle
- Confident navigating systems with good IT skills
