Job Description

Reconnect Navigator - Reconnected to Health
Hours: 37 Hours per week, Monday to Friday between 08 00
Location: HMP Low Newton, Finchale Avenue, Durham, DH1 5YA - Outreach work within the community is also required as part of this role.

Contract: Permanent

Salary: £25,877.80 - £34,240
