Reablement Specialist: Empowering Independence at Home

Walsall Council is seeking a Reablement Officer to support customers in their own homes, delivering active Reablement within a multidisciplinary team and in partnership with NHS Trust and Adult Social Care.

You will coordinate personalised Reablement Plans, assist customers to regain or develop daily living skills, promote independence, and help them access community resources, aiming to reduce ongoing support and maximise confidence.
