Reablement Practice Lead - Social Work Leader (Hybrid)

Slough Borough Council is seeking a Reablement Social Work Practice Lead to support adults and carers to achieve positive outcomes and promote independence. You will provide professional leadership, supervise Reablement Social Workers and handle a small caseload of complex cases.

You'll lead strengths-based assessments, care planning, reviews, and ensure legal compliance and collaboration with therapy teams and external partners for smooth transitions to longer-term care when needed.
