Job Description
As Physician Assistant, you will work as part of amultidisciplinary team using a wide range of skills to provide care to adultmen with chronic and enduring mental health conditions who are residents in alocked psychiatric hospital providing psychiatric rehabilitation services. Theteam is also responsible alongside the associated local GP to care for thephysical health of the residents. Many of the residents have chronic physicalhealth conditions comorbid with their psychiatric conditions.

As the Physician Assistant you will act within yourprofessional boundaries, providing care for residents from initial historytaking, clinical assessment, investigations, care-planning, liaising with otherhealthcare services (principally the associated GP surgery) and ongoingassessment and monitoring of both physical and mental health of the residentsunder the direct supervision of a consultant psychiatrist.

You will work with themulti-disciplinary team to support the delivery of policy and procedures andmeet the needs of residents.

Supervision and mentoring will beprovided by the consultant psychiatrist.

Main duties of the job Provide supervised high-quality clinical care to residentsusing established clinical guidelines delivered in a timely manner

Carry out consultations with residents and consult with theconsultant psychiatrist on diagnosis treatment and referral of residents

Collaborate with nursing staff psychologists occupationaltherapists social workers and other MDT members to deliver coordinated residentcare

Attend and contribute toward ward rounds CPA meetings, andMDT reviews presenting updates on residents physical and mental health progressand care coordination

Support the consultant psychiatrist in collating informationfor clinical decision-making

Contributes and participates in audits evaluation andclinical standard setting

Accurate and timely summarising of resident records

Complete all required paperwork for legal and administrativepurposes in accordance with relevant standards

Produce complete and accurate records of residentconsultation in line with best practice confidentiality policies and procedures

About us Ashness House is a 26-bed inpatient service for males withsevere and enduring mental health illness, co-morbid physical health problemsand complex needs.

It provides specialist MDT assessment, treatment and supportto stabilise a persons symptoms and help them gain or regain the skills andconfidence to live successfully in the community. A holistic approach to careand treatment by a multi-disciplinary team will facilitate assessment,monitoring and review of both mental and physical wellbeing.

Once service users have stabilised, the recovery goal is tomove on to a pathway of community rehabilitation, supported living and thenindependent living. Our aim is to reduce the length of stay, improve functionalwellbeing and general quality of life.

CLINICAL

Make referrals to secondary care and social serviceswithin locally agreed protocols guidance and pathways under the directsupervision of the consultant psychiatrist

Carry out physical examinations investigations and clinicalprocedures within your competence

Instruct and educate residents in preventative health care

Ensure complete and accurate documentation after each residentcontact

Fully document all aspects of resident care and completeall required paperwork for legal and administrative purposes

Carry out necessary invasive and non-invasive diagnostictests or investigations and interpret findings/reports within the scope of aPAs practice Discuss the result and implications of laboratory investigationswith the consultant psychiatrist and with residents

Utilise clinical guidelines and promote evidence-based practice

Offer a holistic service to residents and their familiesdeveloping where appropriate an on-going plan of care/support with an emphasison prevention and self-care

Contribute to quality targets to consistently achieve highstandards of safe evidence-based cost-effective resident care and servicedelivery

ADMINISTRATION

Contributes and participates in audits evaluation andclinical standard setting

Accurate and timely summarising of resident records

Complete all required paperwork for legal andadministrative purposes in accordance with relevant standards

Ensure that all practice policies are fully implemented

Act as a point of contact for communication between theward pharmacy GP surgery and external agencies on physical and mental healthand care management issues

Work in accordance with all governance and internalsystems relating to the management of clinical data and systems

Review and action resident clinical discharge letters undersupervision

TRAININGAND DEVELOPMENT

Taking responsibility for own development with relevantevidence-based knowledge and competence in all aspects of the role to meetclinical governance guidelines for Continuing Professional Development (CPD)and a Personal Development Plan (PDP)

Stay up to date through attendance at any courses and/orstudy days necessary to ensure that professional development requirements aremet demonstrating skills and activities to others who are undertaking similarwork

Subject to a performance review including takingresponsibility for maintaining a record of own personal and/or professionaldevelopment

Work closely with other clinical staff and administrativemanagers in the setting up and/or improving of practice systems formonitoring/measuring performance against Clinical Governance and QualityIndicator targets

Commit to take and pass the necessary Physician Associatere-certification exams (currently every 6 years) to maintain qualificationsrequired to carry out duties of the role and as statutory regulatory bodiesrequire

The Physician Assistant will work under the direct supervision ofthe consultant psychiatrist who retains overall accountability for residentcare

The Physician Assistant will receive clinical supervision andsupport with regular opportunities for feedback and professional development

GOVERNANCE

Produce complete and accurate records of residentconsultation in line with best practice confidentiality policies and procedures

Deliver care according to NHS guidance NICE guidelines andevidence-based care

Take part in the maintenance of quality governance systemsand processes across the Practice and its activities

Utilise the audit cycle as a means of evaluating thequality of the work of self and the team implementing improvements whererequired

Evaluate residents response to health care provision andthe effectiveness of care

Support and participate in shared learning across thepractice and wider organisation

Manage review and identify learning from residentcomplaints clinical incidents and near-miss events

Awareness of statutory safeguarding notification processesand local guidance for children/vulnerable residents applying relevant policiesand legislation to protect them

Ensure compliance with policies procedures and guidelinesfor self and others by acting or alerting senior management team if thepractice appears to contravene policy or if there are concerns over any aspectof the residents care

HEALTH &SAFETY

The post-holder will manage their own and others health& safety and infection control as defined in the Health & Safety PolicyInfection Control Policy and published procedures
