An exciting new opportunity has risenfor a General Practitioner to join our team of forward thinking multi-disciplinaryteam for the PCN which includes Social Prescribing Link Workers, Care Coordnators,Clinical Pharmacists, Pharmacy Technicians, Cancer Care Facilitators, AdvancedNurse Practitioners and Primary Mental Health Nurses.
Those due to obtain their Certificate of Completion of Training (CCT) in the coming months are welcome to apply.
UNFORTUNATELY, WE ARE NOT IN A POSITION TO SUPPORT TIER 2VISA APPLICATIONS.
The successful candidate will beresponsible for working through their own rota and will undertake a variety ofduties including surgery consultations, telephone and video consultations, video,queries and visiting people at home if required. They will be responsible forfollowing up their own investigations and will benefit from regular supervisionand reflection sessions with GP colleagues.
We are committed to the ongoing development of our team, whichincludes mandatory and developmental training for all our staff. Team membersare encouraged to highlight any training they feel would benefit them in theachievement of their role.
Main duties of the jobThe GP willundertake a variety of duties governed by the GMS contract and objectives ofEnhanced Services including:
- Share of routine andurgent appointments both face to face and telephone.
- Prescribe as perlocal & national guidelines.
- Share of home visitsif required.
- Time-managecompletion of administration: letters, results, tasks, referrals, telephonecalls, emails.
- Take responsibilityfor appropriate referrals (following local guidance and good medical practice)and administration pertaining to them.
- Participate inExtended Hours between 6.00pm - 8.30pm if required bythe PCN.
- Work with otherhealth and social care providers to ensure a safe and integrated service.Provide comprehensive assessment, advice, and treatment for the registeredpractice population.
- Maintain accurateand comprehensive records of consultations using SystmOne.
- To provide clinicalsupport to other members of staff, both clinical and administrative, inachieving the highest possible standards of care and QOF and Enhanced Servicestargets.
- The content of thispost will be reviewed in consultation with the post-holder when necessary andin line with the service developments.
Sedgefield North Primary Care Network is an innovative, dynamicPCN combining 5 local GP Practices based in Spennymoor, Ferryhill &Chilton, Sedgefield, and West Cornforth with a population of over 58,000patients. We are continuallyseekingnew ways to support and improve local healthcare services to ourpatient population.
Job responsibilitiesCLINICAL
- Recording clear and contemporaneous I.T.based consultation notes to agreed standards.
- Provide safe, evidence-based,cost-effective, individualised patient care within the hub, surgery, patientsown home.
- Clinically examine and assess patientneeds from a physiological and psychological perspective, and plan clinicalcare accordingly referring patients to other services/agencies in a timelymanner using local guidelines.
- To recognise the need for and be able toperform and interpret investigatory procedures, including laboratory andradiology.
- Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care.
- Ensure appropriate follow up of patients.
- Support patients to adopt healthpromotion strategies that promote healthy lifestyles and apply principles ofself-care.
- Support and work with colleagues towardsthe delivery of anticipatory care plans.
- Recognise and work within your owncompetence and in accordance with professional codes of conduct, maintainingaccurate and contemporaneous health records.
- Independently prescribe for patientswithin your scope of practice. Prescribing in accordance with locally agreed ornational guidelines.
- Review medications following theappropriate policies, NICE/local clinical guidelines and local care pathways.
- To maintain an awareness of developmentsin clinical practice.
- Awareness of and compliance with allrelevant practice/PCN policies/guidelines e.g. prescribing, confidentiality,data protection, health and safety and QOF standards.
- Be aware of your duties andresponsibilities regarding current legislation and adhere to our policies andprocedures on Safeguarding Children and Safeguarding Adults.

You willhave access to confidential information relating to patients and their Carers,practice staff and other healthcare workers. Patients entrust us with, or allowus to gather, sensitive information in relation to their health and othermatters. They do so in confidence and have the right to expect that staff willrespect their privacy and act appropriately.
Informationrelating to patients, carers, colleagues, other healthcare workers or thebusiness of the practice may only be divulged to authorised persons inaccordance with the practice policies and procedures relating toconfidentiality and the protection of personal and sensitive data.
HEALTHAND SAFETY
Thepost-holder will assist in promoting and maintaining their own and othershealth, safety and security as defined in practice's Health & SafetyPolicy, to include:
- Usingpersonal security systems within the workplace according to PCN guidelines.
- Identifyingthe risks involved in work activities and undertaking such activities in a waythat manages those risks.
- Makingeffective use of training to update knowledge and skills.
- Usingappropriate infection control procedures, including maintaining a tidy and safeway work area free from hazards. Effective hand hygiene.
- Provideadvice on the correct and safe management of the specimen's process, includingcollection, labelling, handling, use of correct containers, storage andtransport arrangements.
- Correct useof personal protective equipment (PPE) in both routine and extraordinarycircumstances.
- Managingdirectly all incidents of accidental exposure.
- Safe use ofsharps, storage and disposal.

Person Specification Qualifications- Newly qualified GP (within 2 years and not having had a substantive post) with full GMC registration and license to practice.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Depending on experience£10,000 per session
