Part-Time Social Prescribing Link Worker - PCN

NHS is recruiting Social Prescribing Link Workers to support Primary Care Networks in Waltham Forest. The role is practice-based, not remote, with 18 hours 45 minutes per week on a pro rata basis, and a fixed-term contract of 6-8 months. Probation is 3 months.

The service focuses on prevention, non-clinical support, and reducing health inequalities through social prescribing. Applicants will work within a PCN multidisciplinary team, rotating across practices, delivering person-centered support
