Occupational Therapist (Project Role) - Front Door Services (Children & Adults)
Hybrid working - Office based 3 days, Remote 2 days
Location:Southwark
£31.01 per hour Full-time
Pertemps Social Care are working in partnership with a forward-thinking local authority to recruit an experienced Occupational Therapist to join their Front Door (Contact) Team, supporting both Children's and Adults' services.
This is a fantastic opportunity for someone who is passionate about early intervention and making a real difference at the first point of contact. The role focuses on screening new referrals, completing strength-based assessments, and offering quality oversight to support staff.

Key Responsibilities:
- Screening & Triage
Quickly and effectively screen incoming referrals to determine priority and appropriateness. Liaise with service users, families, and professionals to gather relevant information. - Initial Assessments
Conduct holistic, person-centred assessments in line with the Care Act 2014. Recommend and arrange appropriate equipment or adaptations to maximise independence and prevent unnecessary admissions. - Supervision & Quality Assurance
Support and oversee assistant practitioners and unqualified staff, ensuring high standards across assessments, plans, and documentation. Provide informal mentoring and reflective supervision where needed. - Multi-Agency Collaboration
Work closely with social workers, health professionals, and other agencies to provide coordinated care. Participate in safeguarding discussions, discharge planning, and multidisciplinary meetings. - Professional Documentation
Maintain accurate, timely case records and contribute to audits, service reviews, and ongoing improvements.
Essential:
- HCPC registered Occupational Therapist.
- Strong assessment, communication, and decision-making skills.
- Previous experience within adult social care or community-based OT services.
- Ability to support and guide unqualified or junior team members.
- Experience in front door or rapid response settings.
- Good working knowledge of the Care Act and Mental Capacity Act.
- Familiarity with electronic case management systems.
