Our Medical and Nursing Centers (M/F) - 100% - CMS X medical-social centers (CMS) offer high-quality multidisciplinary care to the residents of Lausanne, based on values of humanity and equity. Peupliers provides day and evening replacements 7 days a week at the 7 CMS X Soins Lausanne to ensure the continuity of care. To complement the team of CMS X, Soins Lausanne is looking for three: Nurse (M/F) with an activity rate between 80% and a permanent fixed contract. Your missions include identifying and analyzing client needs and establishing the initial intervention plan to optimize continuity of care, ensuring clients can return home safely, and coordinating care during their treatment. You will provide care during the treatment period, ensure an on-call service by phone during evenings, weekends, and public holidays, collaborate with network professionals, and respond to their requests. You may need to intervene in acute situations based on your profile and skills.Requirements include a Bachelor's degree in nursing or an equivalent qualification (degree recognized by the CRS), experience in the medical-social field and interdisciplinary work, knowledge of care networks in the canton of Vaud, a sense of responsibility, prioritization skills, and autonomy.
