About the Role

The Leaving Care Advisor will provide tailored support and guidance to care leavers aged 16-25, helping them transition to independent living. The role focuses on empowering young people, ensuring they access the right services, and supporting them in education, training, and employment opportunities.

Key Responsibilities

• Provide tailored support and advice to care leavers aged 16-25
• Build positive relationships and encourage participation in decision-making
• Advocate and explain complex information in accessible ways
• Assess individual needs, identify risks, and set realistic goals
• Manage a caseload and complete assessments within required timescales
• Advise on financial entitlements and ensure care leavers receive support
• Coordinate services with partner agencies (housing, health, education)
• Support access to training, education, and employment opportunities

Candidate Requirements

• Relevant qualification in childcare, education, health, or social care (NVQ 3 or equivalent/higher)
• Strong communication and active listening skills
• Ability to build rapport and maintain professional relationships
• Understanding of trauma and its impact on young people
• Experience in assessing needs and managing risks in complex situations
• Proactive attitude towards helping care leavers achieve independence
