To complete our teams, Soins Lausanne is looking to hire several: Reference Nurse (H/F) Activity rate between 70% and fixed term indefinite - daytime hours with occasional weekends. Your mission: Support clients in their home life project with a view to giving meaning and coherence in close collaboration with their family and loved ones while developing trust-based relationships. Oversee the overall care of clients while relying on internal resources and interdisciplinary teams and coordinating with the institutions involved in the care network. Develop the therapeutic project taking into account the complexity of the care and adapt it according to the evolution. Provide advice, examinations, treatments, as well as nursing and basic care for quality client care. Actively contribute to the sustainable maintenance of clients at home through preventive measures and health promotion while considering individual needs with personalized care and enhancing client autonomy. Participate in development projects and working groups focused on continuous improvement in the field of home care and assistance. Your profile and skills: Bachelor's degree in nursing or equivalent qualification (diploma recognized by CRS). Experience in the medical-social field with a strong interest in community health. Autonomy and decision-making abilities. Ability to collaborate in networks and multidisciplinarity. Sense of responsibility and professional ethics.
