In-Home Reablement Specialist

Walsall Council's Intermediate Care Service is seeking a Reablement Officer (SS02726) to support customers in their own homes, delivering active reablement to restore skills and confidence within a multidisciplinary team.

Working 30 hours per week in Walsall, you will coordinate plans, promote independence and help customers access community resources. NVQ Level 2 Health and Social Care or equivalent is required; excellent communication is essential.
