Hybrid Social Prescribing Link Worker

Nova Wakefield District Limited is recruiting a Social Prescribing Link Worker for the Live Well Wakefield team. The role supports adults 18+ to improve health, wellbeing and independence within Wakefield District Primary Care Networks.

You will identify needs, set wellbeing goals, and connect people to local non-medical services while working in a multidisciplinary team. A driving licence and access to a vehicle are required; Hybrid working is offered.
