Hybrid Short Breaks Assessment & Support Officer

Civic Recruitment Limited is recruiting for a Short Breaks Assessment & Support Officer on a 3 months contract in Nottingham. You will assess the needs of disabled children and facilitate access to Short Breaks Services through a person centred approach, coordinating with families, schools and health professionals.
