Hybrid Family Support Practitioner - Early Help & Hubs

London Borough of Hammersmith & Fulham is seeking a Family Support Practitioner to join our Early Help service within Family Hubs. You will work with children, young people and families, delivering targeted, strengths-based interventions and building resilience through coordinated team efforts.

You will collaborate with Family Hub Navigators and partner agencies to ensure access to universal, targeted and specialist support, with family-centred approaches and robust safeguarding.
