Hospital Social Worker - Discharge & D2A Specialist

Slough Borough Council is seeking a dedicated Hospital Social Worker to join our Hospital Team at Observatory House/Wexham Park Hospital. This permanent, 37-hour role offers hybrid working and a chance to support safe, timely, person-centred discharges within the Discharge to Assess pathway.
