Hospital Liaison WorkerLocation: Barnstaple, Devon
Contract: Permanent
We are excited to offer a Hospital Liaison Worker position working between the Together recovery service and local hospitals.
About the roleThis role involves engaging with individuals admitted to hospital with substance use support needs, using a caring, compassionate, and trauma-informed approach. You will deliver interventions such as advice, harm reduction, and psycho-social support, collaborating closely with hospital staff to ensure appropriate support is provided.
The post involves working within the Together alcohol and drug service to establish a high-quality hospital liaison service, including assessments, interventions, and co-developing discharge plans to prevent relapse and readmission.
Effective communication with hospital departments (e.g., Mental Health, Hepatology, A&E, Maternity) and community services (e.g., GPs, outreach, housing) is essential. You will also provide training and guidance to healthcare professionals in managing substance-related issues, including delivering brief interventions.
Responsibilities- Attend hospital ward rounds and communicate effectively with hospital staff.
- Respond to hospital liaison referrals.
- Deliver interventions face-to-face and over the phone.
- Manage a caseload independently, providing psychosocial interventions.
- Participate in assessments, risk assessments, and referrals within the community as part of a multidisciplinary team.
We seek an empathetic, organized individual passionate about trauma-informed and recovery-focused work. Transferable skills from similar sectors are welcome, with full training provided.
- Knowledge of substance misuse and recovery.
- Experience in managing and prioritizing workloads.
- Understanding of safeguarding.
- Excellent communication, organizational, and time management skills.
- Ability to work collaboratively with diverse teams and organizations.
