Job Title: Hospital Liaison Officer
Reference: Brent
Salary: £18.07 per hour PAYE with accrued holiday and pension contribution
Hours: 36 hours per week Monday Friday
Role length: 3 months opening assignment
Contract Type: Temporary
Location: Brent

Role Description

You will provide clerical, administrative and liaison support to ensure smooth communication between hospitals and Adult Social Care. This includes handling statutory Section 2 and Section 5 notifications under the Care and Support (Discharge of Hospital Patients) Regulations 2014, maintaining accurate records, and ensuring timely and safe discharge processes. The post supports the Reablement and Safeguarding team and plays a key part in maintaining quality, accuracy and customer focused service delivery.

Key Responsibilities
  • Log, track and acknowledge all hospital referrals, faxes and discharge notifications.
  • Liaise directly with hospitals to confirm discharge dates, resolve issues and escalated concerns where required.
  • Act as the first point of contact for all incoming hospital related documentation, ensuring Referrals Officers are promptly informed.
  • Maintain administrative registers, databases and record systems to a high standard of accuracy.
  • Provide statistical information related to hospital discharges as required.
  • Receive and manage telephone and in person enquiries, offering clear information and signposting.
  • Attend case conferences and multi disciplinary meetings, taking minutes and circulating them.
  • Ensure office supplies and relevant equipment are maintained, including hospital fax machine functionality.
  • Carry out photocopying, general administration and cover for other administrative colleagues when needed.
  • Work within the service's Quality Assurance framework, contributing to continuous improvement.
Qualifications (Must)
  • NVQ Level 2 or 3.
  • At least 3 years' experience as an Administrator or Liaison Officer.
  • Strong understanding of the Community Care Delayed Discharge Act and hospital discharge processes.
  • Experience handling multiple tasks in a fast paced environment and working to tight deadlines.
  • Confident communication skills and ability to deal effectively with the public and colleagues at all levels.
  • Proficient IT skills, including databases, spreadsheets, email and general MS Office use.
  • Ability to manage sensitive information and handle challenging enquiries professionally.
  • Strong organisational, prioritisation and numerical skills.
Qualifications (Desirable)
  • Knowledge of services for older people.
  • Experience handling complaints.
  • Understanding of Equal Opportunities and Customer Care standards within a council environment.
