Hospital Discharge Social Worker - Hybrid & Development

Allscreens Nationwide Ltd is recruiting a Social Worker to join the Adult Hospital Discharge Team. You will facilitate safe discharges from hospitals, ensuring appropriate care plans and coordinating with other professionals.

The role offers hybrid working and a clear path for development within a supportive management structure. We require a recognised Social Work qualification, ASYE completed, and a minimum of 2 years post-qualification experience.
