Job Description
To have a good understandingof the Health Coach role; to hold an active caseload and provide support as aTeam Lead to be able to supervise and mentor the Health Coaches. Additionally, to help with the development ofthe Health Coach Team to ensure that the service is robust and sustainable
Healthand Wellbeing Coaches support people to take pro-active steps to improve theway they manage their physical and mental health conditions, based on whatmatters to them. They support people to develop their knowledge, skills,confidence in managing their health and care, to improve their health outcomesand quality of life and support them making changes in health relatedbehaviours.
Healthcoaches do this by coaching and motivating people through multiple sessions toidentify their needs and to set goals for themselves, and through providingaccess to interventions such as self-management education and peer support.
Health and Wellbeing Coaches manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload. They identify when a persons needs are beyond the scope of the health and wellbeing coach role and refer them back to other health professionals or organisations. Health and Wellbeing Coaches may work with people by phone, by video conference, or face-to-face.
Main duties of the job Thesuccessful candidate will be kind, reflective and self-aware and will enjoyworking with a wide range of people. They will have good communication andnegotiation skills and a firm belief that people have untapped resources withinthem, that can be unleashed by providing high quality, non-judgemental support.They will be proactive with a flexible attitude, keen to work as part of a teamand committed to developing themselves in this skilled role and supporting colleaguesto do the same.
Healthand Wellbeing Coaches are a key part of the Primary Care Network (PCN)multidisciplinary team. They often work alongside Coordinators and wider PCNMDT and practices to provide an all-encompassing approach to personalised careand promote and embed the personalised care approach across Primary CareNetwork(s).
Healthand Wellbeing Coaches will have access to ongoing supervision, skillsdevelopment, and support so they are able to further build their skills andexperience within the role.
Pleasenote that the Health and Wellbeing Coach Team Leader role is a hybrid role andthe individual will be expected to maintain an active case load, alongsideproviding support as a Team Leader.

The PCN Teamconsists of Complex Care GP's, Clinical Pharmacists and Pharmacy Technicians,Occupational Therapists, Health Coaches, Care Coordinators, a ServicesDevelopment Lead and a Digital Lead.
The 5 PCNpractices are, Axbridge and Wedmore Medical Practice, Brent Area MedicalCentre, Burnham Medical Centre, Cheddar Medical Centre, Highbridge MedicalCentre.
We have a PCNHub, based at the Burnham Health Campus, Brent Road, Burnham on Sea.
Job responsibilities To work with andsupport the PCNs Clinical Director, PCN Manager, PCN members and their Teams toensure effective delivery of the PCN DES and IIF requirements
Responsible forassisting with day-to-day operational delivery
Support equity ofservice provision across the Networks 5 core practices
Ensuring objectives ofteam is being delivered across all 5 Practices, and when staff absences occur ensurethat activities are maintained and delivered.
Trouble shooting day today and responding to any queries from Health Coaches and wider team asrequired.
Develop and maintaineffective communication both with the ARRS staff and with all partnerpractices.
To provide support withproject implementation and communications for new services.
Support the PCN todevelop/ maintain a patient voice forum, patient facing comms and social mediapresence for the PCN
Organise meetings orevents related to PCN activity.
Attend and present atPCN Executive Board and Clinical Leads Meetings
Where required, take alead role in project delivery. This could range from individual PCN projects tosupporting clinicians in the development of new care pathways.
Help develop jointworking and relationships across networks and its stakeholders.
Communicate informationand issues, including briefings and reports.
Maintain excellentrelationships across all core member Practices.
To produce, review andcirculate documents required for meetings.
Minute meetings.Collating action points and circulating.
Supporting PCN memberswith all other reasonable administration requests.
Supporting andcollating evaluation of PCN services to drive further improvements.
Help develop policiesand procedures that are accurate.
Support the financialdelivery of PCN activities.
Develop and maintaindatabases required for the role.
Work with the PCNmember practices to identify and address access barriers for those who struggleto access healthcare.
Working with other PCNHealth Coaches and Care Coordinators, raise awareness of health promotion,screening, NHS Health Checks and LD Health checks with patients.
Work flexibly, adaptingto the needs of the service and client group while maintaining the integrity ofthe role.
HR Duties
To undertake day-to-dayline management of all Health Coaches across the PCN.
Support the retention ofstaff and ensure HR needs are met for all Health Coaches.
To provide line management to other Health Coaches; including management of absence line, managingand arranging cover for staff as required.
Support the recruitment processes for new PCNstaff, from advertising to appointment, as required.
Arrange and oversee staffinduction and training, ensuring staff have completed all training
Conduct appraisals andsupport performance management of designated PCN staff as agreed.
Deliver a high standard ofpatient care, leading Health Coaches within the network, to workcollaboratively with the PCN members to meet the needs of patients.
Personal and ProfessionalDevelopment
Keep abreast of nationaland local guidance on PCNs.
Take responsibility for owndevelopment, learning and performance.
Participate in an annualindividual performance review, and
takeresponsibility for maintaining a record of own personal and professionaldevelopment.
Actively work towarddeveloping and maintaining effective working relationships both within andoutside the practice, PCN and locality.
Demonstrate ability tointegrate general practice with community and hospital teams.
Liaise with otherstakeholders as needed for the collective benefit of patients.
