Job Overview Diabetes Patient Educator and Technology Support

This patient facing role is suitable for someone who enjoys educating and caring for others and is competent with communication technology. Ability to work independently is also important. The successful candidate will be required to adapt training and support according to patient age and ability. Some patients may require repeat appointments until they are comfortable and competent with the technology.

Main Duties of the Job To work independently as part of the diabetes team, under the leadership, supervision and guidance of the Diabetes Specialist Nurses, in the support of people with diabetes in the hospital setting.
Manage a delegated caseload. Provide education to patients (one-to-one and in groups) to promote self-management of diabetes.
Encourage and support people with diabetes to manage their condition more effectively with the use of glucose monitoring systems (worn technology) and smart pens. The role will include training on glucose monitoring, smart pens, mobile phone apps, email, enabling the sharing of data and supporting with any technical issues patients may have.
Responsible for service improvement initiatives. Collate and analyse the results from surveys and questionnaires to inform improvement work and present this in a range of formats for different staff groups/audiences.
Support with ensuring data sharing prior to/at planned clinic appointments and teaching patients how to do this from home where possible.
Support with technology groups and maintenance of spreadsheets/record keeping to include GP correspondence, communication with patients/family, company representatives, finance and payments teams.

Working for our Organisation We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our Community Service for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.

Detailed Job Description and Main Responsibilities The post holder is responsible for the operational management of the diabetes technology administration processes and functions. They will provide a professional and comprehensive Diabetes Technology Administration Service across the Trust. Work in partnership liaising with patients, outside agencies, companies and MDT members to ensure timely access to diabetes technologies. Manage the day to day set up and delivery of diabetes technologies for people living with diabetes. Encourage and support people with diabetes to manage their condition more effectively with glucose monitoring systems and smart pens. Provide training on glucose monitoring, mobile phone apps, email downloads and video call systems to patients. Work alongside the diabetes team and in harmony with all members of the MDT to ensure patients receive an efficient service. Forge positive working relationships with all staff within and outside the Trust involved with the diabetes service. Possess a strong understanding of the role of partner technology companies.

JOB SUMMARY

11. Attend meetings as and when required, preparing agendas, and may include taking notes and preparing minutes, both formal and action plans.
12. Responsible for maintaining accurate patient and staff records in line with Trust guidelines.
13. To be involved in monitoring and evaluation of patient feedback, offering recommendations as part of the team to improve the service.
14. Responsible for audit within own work area.

RESPONSIBILITIES
1. To work within own sphere of competence and within agreed protocols.
2. To undertake agreed clinical activities with patients under the direct and indirect supervision of the DSN team.
3. Alert line manager to any issues in work and maintain appropriate levels of confidentiality.
4. At all times treat patients, visitors and colleagues with dignity and respect, exercising discretion and maintaining confidentiality.
5. To work at the pace of the patient with explanation in understandable language.
6. Flexible response to patient difficulty with technology or equipment.
7. To demonstrate and support the setup of glucose monitoring systems, sensors, readers and apps.
8. To demonstrate how to use glucose sensors and meters, how to download and send clinical information so that it is accessible for clinic appointments.
9. To make relevant and necessary decisions on substitute meters, finger-pricking and glucose reading devices.
10. To support with HbA1c measurements once trained and capillary monitoring where appropriate.
11. To practice in a patient-centred way that supports physical and psychological wellbeing and facilitates patient self-management.
12. To offer telephone, email, virtual and face to face support as needed.
13. To provide educational material and advice to patients to ensure continuity of care.
14. To exchange effective communication with patients, family and carers.
15. To liaise when necessary with the DSN's and the wider diabetes team. 1
16. To report any accidents, incidents or other relevant issues to the DSN.
17. To participate and contribute to departmental MDT's.
18. To maintain stock of equipment and materials and to ensure quality control and calibration of equipment.
19. The post holder will be required to demonstrate the ability to use a variety of IT systems and other systems needed for the downloading and interpretation of monitoring data.
20. The role will require liaising with medical companies, finance, MDT members and external bodies in order to support with account set up, organising training sessions and ensuring that all records are updated. Maintenance of technology databases is also required.
21. To support the process of continuous quality improvement, assisting in audits, team meetings and service improvement.
