£59,320 per annum, plus £3,000 car allowance

Contract type: 40 hours per week, predominately Monday to Friday but must be flexible

About the roleAs a Dementia Specialist Nurse with Exemplar Health Care, you'll lead, develop and advance dementia practice across Exemplar Health Care, ensuring outstanding, evidence based, person centred care for people living with dementia.
No two days will ever be the same, but your day to day responsibilities will include:
leading the organisation's dementia strategy in alignment with national policy and best practice
mapping dementia needs across homes and developing specialist pathways
strengthening external relationships with NHS and dementia organisations
designing and delivering dementia training programmes
developing and supporting a network of Dementia Champions
providing specialist triage, consultation and virtual clinic support
leading complex dementia assessments and risk formulations
developing dementia policies, audits and assessment tools
leading quality reviews and improvement initiatives.
About youAbove all, you're someone who identifies with our Exemplar Health Care values of fun, integrity, responsiveness, success and teamwork.
You must also have a first level nursing registration with a valid Nursing and Midwifery Council (NMC) PIN.
As this is a high level, strategic role, you should have a minimum of two years' experience in a Dementia Specialist Nurse role.
You'll also have:
evidence of extensive CPD in dementia care
evidence of further professional development in relation to care, management or quality
ability to coach and motivate others
a strong background in action planning
excellent written and verbal communication skills
a pragmatic approach, with strong problem solving and analytical skills, along with the ability to apply these in complex situations
the ability to work collaboratively with, and travel to all locations across the business.
