Job Description

Hours: Full Time

Location: South Gloucestershire

Contract Type: Permanent

The Role

This role focuses on delivering person centred support by welcoming individuals, providing guidance, and helping them access appropriate services. The post holder will carry out assessments and develop tailored support plans that build on each individual's strengths, needs, and aspirations, supporting them to achieve positive and sustainable change. They will monitor progress, maintain accurate and confidential records, and advocate for clients where necessary to ensure their needs are met.
