Job Description 3 Month Contract With A local Authority

Job Purpose

To act as the subject matter expert for Adult Continuing Healthcare (CHC), supporting the development and maintenance of high-quality practice standards in line with national legislation, guidance and local policies. The postholder will provide strategic and operational support to managers and social workers, offering specialist advice, training, consultation and practical support on complex CHC matters, disputes and funding processes.

Key Responsibilities
  • Act as the lead specialist for Adult Continuing Healthcare (CHC) across Adult Social Care services.
  • Provide professional advice, guidance and training to managers, social workers and wider Adult Social Care teams on CHC processes, legislation and best practice.
  • Complete CHC Checklists and Decision Support Tools (DSTs) in line with national guidance and local procedures.
  • Gather, analyse and present evidence in support of CHC applications and funding requests.
  • Support and represent the local authority in CHC disputes, appeals and funding refusal cases, including attendance at Independent Review Panels and Ombudsman proceedings where required.
  • Contribute to the development and implementation of policies, procedures and service improvements relating to CHC practice.
  • Review and monitor ongoing care and support plans in partnership with colleagues, providers and partner agencies to ensure effective outcomes and value for money.
  • Support the professional development of staff through supervision, coaching, consultation and practice support.
  • Participate in quality assurance and audit activity to maintain high standards of practice and accurate record keeping.
  • Monitor and maintain accurate CHC records using Adult Social Care systems and databases.
  • Support budget monitoring processes and assist in identifying service and resource pressures.
  • Assist with Freedom of Information (FOI), Data Protection Act (DPA) enquiries and complaints processes as required.
  • Work collaboratively with health partners, providers and internal departments to ensure effective integrated working.
  • Undertake additional duties appropriate to the grade and responsibilities of the post
Requirements
  • Experience working within Adult Social Care, Continuing Healthcare or a related health and social care setting.
  • Strong knowledge of Continuing Healthcare legislation, frameworks, Care Act 2014 and relevant statutory guidance.
  • Experience completing CHC Checklists and Decision Support Tools (DSTs).
  • Experience supporting complex casework, disputes and appeals relating to CHC funding.
  • Strong analytical, organisational and case management skills.
  • Ability to provide professional advice, training and support to staff and stakeholders.
  • Experience contributing to audits, quality assurance and service improvement initiatives.
  • Strong communication and partnership working skills.
  • Ability to maintain accurate records and manage sensitive information confidentially.
  • Competent in the use of case management and IT systems.
