Role OverviewPermanent post, 40 hours per week, based within the specialist Older People and Stroke Unit at the Royal Alexandra Hospital, Paisley. The position focuses on front door frailty and the delivery of Hospital at Home services, expanding acute frailty pathways and supporting alternatives to hospital admission.
Responsibilities- Oversee provision of acute medical care to patients in their own homes.
- Lead a multidisciplinary team to deliver patient centred Hospital at Home care.
- Develop and implement frailty pathways and comprehensive geriatric assessment (CGA) initiatives across acute, virtual and community settings.
- Foster collaboration with acute, virtual and community services to facilitate early supported discharge.
- Champion quality improvement and education within the service.
- High level of clinical experience in all aspects of geriatric medicine.
- Strong communication and leadership skills.
- Ability to adapt to varied patient needs and settings.
