Consultant in Diabetes & Endocrinology

This is a replacement post for a 10 PA Endocrinologist within the department, following the retirement of Dr Alison Evans. The role focuses predominantly on endocrinology, complemented by one general Diabetes clinic per week. The successful candidate will lead the Joint Pituitary Service, conducting bimonthly clinics with the Southmead Hospital neurosurgical team. This involves managing complex cases in coordination with the BNOG Skull Base MDT and the QEH Birmingham pituitary service. Additionally, the appointee will participate in the local NET MDT, collaborating with the Tri-Counties Oncology Service, UGI surgeons, and radiology, while liaising with the Birmingham National Centre of Excellence as needed. As a large District General Hospital (DGH), GHNHSFT manages an endocrine caseload of significant volume and complexity. The service is supported by a robust MDT structure, including a monthly Biochemistry MDT and a quarterly Neuroradiology MDT featuring a visiting specialist from NBT. We are seeking a candidate to enhance our clinical pathways, specifically strengthening the link with ENT for the management of Head and Neck paraganglioma.

Main duties of the job

This substantive Consultant post is for the Trust's Diabetes & Endocrinology service. As a senior clinician, the post-holder will work collaboratively with medical, professional, and managerial colleagues to deliver high quality healthcare across the Trust's patient population. The role includes a commitment to the weekend acute medicine service (currently 1-in-12), conducting post take ward rounds for unselected medical takes. Notably, there is no general medicine on call commitment. GHNHSFT serves as a major clinical hub, providing a full primary PCI service, an out of hours GI bleed rota, SDEC, and stroke thrombolysis. As the regional vascular hub, the Trust co locates vascular surgeons with an inpatient bed base; the successful candidate will be expected to cover 10 specialty inpatient beds on this shared ward at GRH on a 2-in-12 basis. In alignment with the NHS Long Term Plan, the appointee will work cross site and at community hospitals to provide care closer to home. Our endocrine service is one of the largest users of Advice & Guidance (A&G) in the country according to the latest GIRFT data, and the successful candidate will be expected to contribute actively to this workload. A collaborative, team oriented approach is vital. The post holder must take ownership of the department's general workload, providing essential cross cover for A&G, ward rounds, and inpatient referrals during annual leave or colleagues' non working days.

Job responsibilities

1. Clinical Responsibilities / Key Responsibilities - The appointee will be expected to share responsibility for the delivery of high quality care to the patients presenting to the Department; support the clinical decision making of junior medical and nursing staff; provide on call support to the general medical take out of hours; attend out of hours as agreed protocols; remain contactable and available when on call; carry out ward rounds.

2. Professional Development, Education and Training - The duties of this post will also include supporting primary care to manage the large majority of patients with diabetes who do not require secondary diabetes care input. This will involve liaising with GPs and Practice Nurses, taking part in education programmes for practice staff, responding to email requests from GPs and Practice Nurses for clinical advice, and taking on the supervision of the GP vocational training post. Integrated Diabetes Team: Education, supervision, and regular appraisal.

3. Management and Leadership - To provide medical information for the development of systems appropriate for Trust needs; participate in departmental consultant and senior staff meetings whilst remaining within the framework of the agreed strategy of the Trust and the Department; attend other departmental, Divisional and Trust meetings as necessary; attend regional and national meetings as necessary; undertake all work in accordance with Trust procedures and operating policies.

4. Quality Improvement - To promote evidence based practice; respond appropriately to NICE guidance; change own and others clinical practice in response to new clinical developments; support the development of weight sensitive policies across Gloucestershire; undertake and supervise relevant clinical audit topics; be involved in risk management and quality assurance; remain up to date and engaged in CME/CPD as per RCP guidelines; respond appropriately to complaints and critical incidents. The post holder will be expected to take part in developing clinical audit in accordance with agreed Directorate and Corporate Clinical Governance programmes. Teaching primary care colleagues, GP trainees, junior doctors and medical students, nurses and other paramedical staff in the team in speciality and special interest.

5. Accountability - The post holder will be immediately accountable to the specialty Clinical Lead / Specialty Director. Further line management is provided by the Chief of Service, following medicine and subsequently the Medical Director. Co operate with the commissioning lead on budget issues. The intention is to have a programme budget for diabetes across primary, community and secondary care.
