This position will be based at Weston General Hospital which accommodates 279 inpatient beds, including a Medical Assessment Unit and separate Older Persons Assessment Unit and an Emergency Department with an award winning embedded Geriatric Emergency Medical Service (GEMS).

We are seeking an enthusiastic Physician to continue the developments within Care of the Elderly to support the Trust in improving how it handles patients arriving for potential admission with the aim of delivering timely safe care as well as increasing the numbers of early discharges. This post holder will be part of the team that supports frailty admissions pathways and will also have an instrumental role in shaping the future of the Parkinson's Disease and Movement Disorder Service across Weston-super-Mare and Worle.

Main duties of the job The appointee will be responsible for the development of Care of the Elderly services along with their colleagues. The key objectives are to reduce number of unnecessary admissions, reduce patient length stay and work with system partners to ensure safe discharge to appropriate places of residence. There will be a strong commitment to teaching Junior Staff the principles of management of elderly patients, this will occur as the opportunity arises on a day to day basis with acute patients, as well as dedicated teaching. Job responsibilities This post will help to continue to facilitate the progression of Care of the Elderly services. It is an exciting time to be joining our team we are ambitious to further develop the model of care for frail elderly patients including our services in areas such as Frailty, Orthogeriatrics and Dementia - we are keen to support the successful candidate in specialist interest areas. At Weston our 12 Care of the Elderly physicians work alongside a well-developed frailty team made up of Therapists, Advanced Practitioners, Specialist Nurses, Ward Nurses and Pharmacists to deliver a comprehensive front door service (including Frailty Same Day Emergency Care), an Older Persons Assessment Unit, three ward areas and outpatient clinics including Parkinson's Disease and Movement Disorder and a Rapid Access Care of the Older Person (RACOP) clinic. The appointment of this position will be part of the 1:11 frailty consultant on call rota. The main site for this post at present is Weston General Hospital but as we move to full clinical service integration across UHBW it is anticipated that consultants could have some element of cross-site working to be determined through the job planning process.

Person Specification Qualifications & Training
  • Full Registration and a licence to practise with the General Medical Council (GMC)
  • Entry onto the General Medical Council (GMC) Specialist Register in Geriatric Medicine and General Internal Medicine (GIM) or
  • Eligible for specialist registration within six months of interview (please note, this does not apply to CESR) or
  • Holder of Certificate of Completion of Training (CCT), or evidence of within six months of award of CCT or equivalent by date of interview
  • Higher academic qualification e.g. MD, PhD or equivalent
  • Educational Supervisor accreditation
  • Advanced communication skills training
  • Good Clinical Practice training
Experience
  • Evidence of thorough and broad training and experience in medicine for older people
  • Able to take full responsibility for delivering service without direct supervision
  • Proven ability to work well within a multidisciplinary team and support others to flourish
  • Ability to respond sensitively to the individual needs of patients and to the needs of carers, relatives and friends
  • Specialist interest in Parkinsons and Movement disorders
  • Ability to lead clinical care across professional and organisational boundaries
  • Evidence of special interest and expertise that complements those of other consultants in the department and is consistent with the Trust's service strategy
  • Experience of drafting or contributing to multi-professional/multiagency clinical guidelines or care pathways
Clinical Knowledge and Skills
  • Knowledge of NHS priorities with reference to specialty
  • Ability to organise efficient and smooth running specialist service. Proven ability to organise and prioritise workload
  • Broad range of IT skills
  • Specific skills or qualifications in relation to Parkinsons and movement disorders
Clinical Governance and Audit
  • Understanding of clinical governance and the individual responsibilities it implies
