The Consultant Practitioner for Home Treatment Service leads the Home Treatment Service alongside the SAS Doctors West Kent.
We are a community team made up of a variety of staff including Specialist Doctors, Speciality Doctors, Advanced Clinical Practitioners and Health Care Assistants.
The Home Treatment Services have excellent links with Primary Care, Community Teams, Hospice, SECAMB Trust and Acute Trust Teams.

Main duties of the jobThe post holder will be a clinical expert in Frailty and will fulfil the 4 domains of Advanced Clinical Practice:
- Advanced and expert level clinical practice within the field of frailty ensuring clinical excellence
- Leadership and consultancy providing expert professional advice to patients, carers and colleagues
- Development and provision of education and training to other staff and trainees
- Service development, evaluation, research and audit.

Person Specification Qualifications- First level Registration with NMC, HPCP or GPHC
- Demonstratable, Significant or Extensive post registration experience
- Teaching qualification
- Research qualification
- Non- medical Prescribing
- Psychology or counselling experience and training/ education
- Extensive clinical experience in the care and treatment of people with frailty (including long term conditions and dementia care experience)
- Advanced skills in history taking and physical assessment of frail people
- Independent prescribing experience
- Experience of advanced specialist interventions in the assessment and management of older patients
- Experience of planning, implementation and delivery of educational programmes relating to long term conditions
- Extensive experience of presenting report/educating/ information to public forums
- IR(ME)R training or equivalent
- Advanced understanding of long-term conditions and application to older people
- Advanced knowledge of clinical governance
- Advanced knowledge of change management
- Advanced knowledge of managing complex patient information and situations
- Extensive research knowledge
- Demonstrate a high level of competence and excellence in practice
- High level competence in strategic planning
- Advanced skills in writing reports
- High level skills in presenting
- Coaching and mentoring skills
- Peer review of research or publications skillsRisk management skills at a high level
