Compassionate Supported Living Advisor (12 Month Contract)

Bracknell Forest Council is seeking a Supported Living Advisor on a fixed-term contract for 12 months to provide practical support and housing-related assistance to a caseload of referred service users, on-site in Bracknell Forest. You will ensure service users are actively involved in their agreed care plans, supporting access to services and facilities, including transport where appropriate.
