Community Paramedic: Proactive Home Care Specialist

Deal and Sandwich PCN is seeking an experienced Paramedic to provide patient-centred care across patients' homes, care homes and community settings within Deal and Sandwich PCN. You will assess, treat and manage health needs while supporting proactive care to reduce hospital admissions.

The role operates within the EHCH framework, liaising with the PCN team and external services to deliver integrated primary and community care.
