Community Nurse - Home Care, Wound & Palliative Expert

CPFT is recruiting a Community Nurse (Band 5) to deliver high quality care in patients' homes across Cambridge North City and Villages. You will be supported to develop wound care, palliative care and long-term conditions management, with emphasis on assessment and care planning to meet individual needs.

The role requires a full UK driving licence and access to a vehicle, as you will travel around Cambridgeshire meeting tight deadlines and providing care in community settings, with a strong
