Clinical Specialist Practitioner - Crisis Response Team

Are you an experienced practitioner who would like to work within a community based MDT, having a passion for supporting patient care and keen to make a difference? We are looking for an enthusiastic Clinical Specialist Practitioner who wants to join an established admission avoidance service to support patients to stay at home where possible, reducing the need for hospital admission.

The Clinical Practitioner will work primarily in our Urgent Care Crisis Response service but will have the opportunity to support the Hospital at Home service. They will provide clinical assessments at home working closely with our ACPs, receiving referrals from community services, NWAS, 111, GPs and facilitating ED deflections with a focus on keeping patients safe at home where possible.

As a confident communicator with strong organisational skills, you will thrive in an MDT environment and will be instrumental in supporting and developing this service to ensure a high quality service for our patients. With a passion for continuous development and service improvement, you will be involved in projects and trust initiatives, receiving regular supervision, in service training and support.

Main duties of the job

Undertake comprehensive health care needs assessment of patients, reassessing as appropriate.

Assess, plan, prioritise, implement and evaluate interventions to meet patients' needs.

Contribute to the organisation of multi disciplinary, patient centred care.

Facilitate patients, relatives and carers to take an active role in care given, providing information as appropriate to remain safe at home.

Using your experience and knowledge, you will support other team members to ensure we deliver a high quality service for our patients, providing education and support as needed.

You will also have the opportunity to work across hospital and community services whilst developing key links within primary care and the wider networks within acute and community.

Person Specification Qualifications
  • NMC / HCPC Registration
  • V300 Non Medical Prescriber
  • Evidence of relevant post registration training / CPD
  • Experience in Urgent Care Delivery
  • Accredited clinical assessment skills
Experience
  • Broad experience at Band 6 working in community setting / Rapid Response / Intermediate care
  • Experienced in providing clinical leadership and problem solving.
  • Experience of working within a team and as an autonomous practitioner.
  • Experience of urgent assessments and developing and delivering care plans to meet these needs.
  • Substantial post registration NHS experience.
  • Evidence of involvement in change management.
  • Evidence of involvement in policy and practice changes arising from audit.
Knowledge
  • Knowledge and understanding of complex medical, social and psychological needs of older adults.
  • Able to demonstrate a sound knowledge of Clinical Governance and risk and its implications for the Service.
Skills & Abilities
  • Excellent interpersonal and communication skills.
  • Able to lead and motivate a team and implement change.
  • Evidence of expert clinical practice.
  • Clinical reasoning skills / knowledge underpinning clinical skills specific to clinical area.
  • Ability to effectively manage / delegate others' workload.
  • Excellent organisational skills.
  • Ability to work in unpredictable environment e.g., manage crisis and reorganise workload at short notice.
