Job Description

Oakham Medical Practice and Market Overton and Somerby Surgeries have the exciting opportunity to recruit a Practice Pharmacist.

We can provide a stable and supportive background for newly qualified Pharmacists and those in their first 5 years or equally the opportunity to grow and develop if you have more experience. The role will be based across the three Practices, with flexibility to work from home (hybrid).

We are a dynamic, creative and forward thinking Partner ledteam, training practice for GPST1-GPST3s and have supported numerous ACPs andPharmacists throughout their training. This new role will be integral to ourmedicines management team and involve working to provide a safe medicationservice as well as innovatively managing our patients with long term conditionsas part of our multi-disciplinary team.

The Practice uses SystmOne and operates Monday to Friday.

To work as an autonomous practitioner, within theirprofessional boundaries, as part of the Practice multidisciplinary team. Theywill be supported by the Partners and Lead Pharmacist.

The role is pivotal to improving the quality of care andoperational efficiencies so requires motivation and passion to deliverexcellent service within general practice.

Oakham Medical Practice is based in a thriving market town in the beautiful county of Rutland. Market Overton is a picturesque village in the North of Rutland with a branch surgery in the village of Somerby just across the border in Leicestershire. The Practices serve 20000 patients as well as providing medical cover for the local public school in Oakham.

Job responsibilities

To work as an autonomous practitioner within theirprofessional boundaries as part of the Practice Multidisciplinary Team. Theywill be supported by the Partners and Lead Pharmacist.

The role is pivotal to improving the quality of care andoperational efficiencies so requires motivation and passion to deliverexcellent service within General Practice.

Responsibilities

Reauthorise repeat prescribing by reviewing patientrequests, medicines reaching review dates and flagging up those in need of areview.

Ensure patients have appropriate monitoring tests inplace.

Provide telephone advice or face to face clinics topatients with questions, queries and concerns about their medicines and acutemedication requests.

Reconcile medicines from clinical letters and followingdischarge from hospitals, identifying and rectifying any changes.

Work with patients face-to-face or on the telephone whohave multiple medical conditions where medicine optimisation is required andreview the on-going need for each medication i.e. Long Term Conditions.

Undertake clinical medication reviews, includingstructured medication reviews, with patients.

Answer relevant medication related enquiries from GPs,other practice staff, other healthcare teams, hospitals, District Nurses andcommunity Pharmacies.

Undertake clinical audits of prescribing in areas directedby the GPs and ICB, feedback the results and implement changes in conjunctionwith the Practice Team.

Implement changes to medicines that result from MHRAalerts, product withdrawal and other local and national guidance.

Work with the Team to ensure the Practice iscompliant with CQC standards.

Person Specification Experience
  • We can provide a stable and supportive background for newly qualified Pharmacists and those in their first 5 years or equally the opportunity to grow and develop if you have more experience.

Knowledge
  • Providing clinical expertise and being an educational resource for colleagues in relation to medicines eg doses, side-effects, alternatives etc whilst helping to address the public health and social needs of patients within NICE and other evidence-based guidelines
  • Good knowledge, awareness and understanding of relevant professional and clinical standards and guidelines.
