Summary:

We are currently seeking a dedicated Clinical Pharmacist to join our team at NHS Jobs. This is a permanent, full-time position based in Canterbury, CT1 1AZ.

Responsibilities:

• Developing close working relationships with other members of the multi-disciplinary team.
• Contributing to clinical care plans by providing insights into medication management.
• Responding to medication-related queries and offering expert advice on prescribing practices.
• Promoting evidence-based practice to ensure optimal patient care outcomes.
• Engaging with patients and their carers to discuss medications, providing information and support.
• Developing approaches to facilitate informed care in the community.
• Educating colleagues about medicines management and promoting medicines optimization.

Qualifications and Skills:

• A valid Pharmacist qualification and registration with the appropriate regulatory body.
• Proven experience in a clinical pharmacy setting.
• Strong communication and interpersonal skills.
• Ability to work effectively within a multi-disciplinary team.
• A commitment to evidence-based practice and continuous improvement.
