Job Description
Clinical Pharmacist in Fairford (Part Time - 3 month contract) Hilary Cottage Surgery is nestled in the South Cotswolds. A small surgery with around 40 staff, this part time, temporary role is funded by our PCN.

Your temporary role role here will be towork alongside the GPs and other healthcare professionals to provide expertise in clinicalpharmacy, medicines optimisation and safety which will include undertaking structured medicationreviews. You will provide leadership for medicines use toensure patients experience high quality patient care that will improve health outcomes andefficiency of the primary care team by providing direct, accessible and timelyexpertise including telephone contact with patients.

The main duties and responsibilities will include performing remote medication reviews, support management of patients withlong-term conditions, undertake medicines reconciliation, provide telephoneconsultations, repeat prescription standardisation, implementation andmanagement, drug monitoring management, and clinical audits.

Main duties of the job Duties include:

Action medication adjustments following hospital discharge summaries and identifying and rectifying unexplainedchanges to ensure patients are supported and receive correctmedications and deal with any queries.

Manage medication reviews withpatients by telephone and make recommendations for prescribing and monitoring. Through structured medical reviews the postholder will support patients to take their medications to get the best fromthem, reduce waste and promote self-care. Medication reviews for patients whoare regular A+E, Urgent Care Centre or hospital attenders.

Carry out clinical audits to review patients onhigh-risk medications and monitor prescribing against local and nationalguidelines.

Make recommendations for medicine switches andsupport staff with medicine related queries.

Work with the prescribing lead to review repeatprescribing policy. Make necessary changes and interventions forre-authorisation of long-term medications.

Management of repeat prescribing and managementof patients with chronic diseases where medicine optimisation is required.

Promote and increase uptake and utilisation ofelectronic repeat Dispensing (eRD) and Electronic Prescribing (EPS) in linewith defined targets.

Assist the practice to achieve QOF and EnhancesService objectives/ targets.

Provide advice and support to patients in theuse of medicines.

Promote and increase uptake and utilisation ofelectronic repeat Dispensing (eRD) and Electronic Prescribing (EPS) in linewith local & National targets.

Assist the practice to achieve QOF and EnhancesService objectives/ targets.

Provide advice and support to patients in theuse of medicines.

Knowledge, skills and experience required

Completion of an undergraduate degree inpharmacy and registration with the General Pharmaceutical Council.

Must be a qualified independent prescriber orworking towards

Have experience and an awareness of commonacute and long-term conditions that are likely to be seen in general practice.

Recognise priorities when problem solving andidentifies deviations from normal pattern and is able to refer as appropriate.

Able to follow legal, ethical, professional,and organisational policies and procedures and codes of conduct.

Involves patients in decisions about prescribedmedicines and supporting adherence as per NICE guidelines.

Administration and professionalresponsibilities

Lead on and develop elements of the clinicalpharmacy service to improve the patient experience.

Contribute proactively to ensure optimalmedicines usage by patients and healthcare professionals.

Contribute to ongoing development and serviceimprovements in conjunction with the multi-disciplinary team.

Participate in the administrative andprofessional responsibilities of the Practice Team

Ensure accurate and legible notes are recordedin the patients computer record

Ensure accurate completion of all necessarydocumentation associated with patient care

Ensure collection and maintenance ofstatistical information required for regular and ad-hoc reports and audits

Attend and participate in Practice meetings asrequested

PCN objective such as IIF and practicepriorities such as QoF and CQC requirements
