We areseeking an enthusiastic and patient-focused Clinical Pharmacist to joinour team and play a key role in optimising medicines use, improving patientoutcomes, and supporting our GP and nursing colleagues.
TheClinical Pharmacist will work as an integral member of the practice clinicalteam, with a strong emphasis on direct patient care, medicationreviews, and long-term condition management. The role involves closecollaboration with GPs, nurses, and other healthcare professionals to ensuresafe, effective, and evidence-based prescribing.
Main duties of the jobPatient Care & Medication Optimisation
- Conduct structured medication reviews, including for patients with complex needs
- Provide patient-facing clinics (telephone, and remote)
- Support medicines optimisation to improve safety, adherence, and clinical outcomes
- Manage medication-related queries from patients and clinicians
- Support deprescribing where appropriate, in line with national and local guidance
Long-Term Condition Management
- Support management of chronic diseases such as hypertension, diabetes, asthma, COPD, and cardiovascular disease
- Work with the nursing team to support care planning and treatment
- Monitor and interpret clinical results related to medicines management
Prescribing & Clinical Support
- Prescribe independently (where qualified) within scope of practice
- Support repeat prescribing processes and reduce prescribing workload
- Review and action hospital discharge summaries and medication changes
- Contribute to audits, QOF, and prescribing quality initiatives
Multidisciplinary Team Working
- Work closely with clinical team
- Provide medicines-related advice and education to the wider team
- Participate in practice meetings and service development
Governance & Professional Development
- Maintain accurate and timely clinical records
- Work in line with NICE guidance, local formularies, and best practice
We are alarge, well-established GP practice in Stockport serving approximately 18,000patients over 3 sites. Our multidisciplinary team includes GPs, nurses, HCAs,pharmacists, pharmacy technicians, and wider PCN colleagues. Person Specification Experience- Experience working in primary care, general practice, or a patient-facing clinical role
- Experience of structured medication reviews and polypharmacy
- Experience working within a GP practice or PCN
- MPharm degree or equivalent
- Registered with the General Pharmaceutical Council (GPhC)
- Experience working in primary care, general practice, or a patient-facing clinical role
- Strong clinical knowledge and commitment to patient-centred care
- Excellent communication and team-working skills
- Independent Prescriber qualification (or working towards)
- Knowledge of EMIS
- CPPE Primary Care Pharmacy Education Pathway completed or in progress
