Main duties of the jobSupport to general practice staffwith regards to prescription and medication queries
Medicines reconciliation on transferof care and systems for safer prescribing
Expertise in clinical medicinesadvice while addressing both public and social care needs of patient in the GPpractice
clinical leadership on medicinesoptimisation and quality improvement

Qualifications- Experience in General Practice
- Completed or enrolled in CPPE Pathway
- Able to run chronic disease clinics
- Offer Medicines advice to patients and clinical staff
- Undertake audit activities and discussions with the General Practice team
- Have a Masters in Pharmacy and a minimum of 2 years post reg experience
- Independent Prescribing Pharmacist: 1 year (required)
- Completed or enrolled in CPPE Pathway
- Experience in General practice - dealing with patient medical reviews
- Medicines reconciliation on transfer of care and systems for safer prescribing
