We are seeking dedicated and knowledgeable Clinical Pharmacists to join our healthcare team. The ideal candidates will play a crucial role in ensuring the safe and effective use of medications for patients. This position requires a strong foundation in anatomy, medication administration, and patient care to provide optimal pharmaceutical services. These roles are full/part time and a mix of remote and practice based working.
Responsibilities- Completing medication reconciliations and reviews for patients SMR both in person and over the telephone
- Medicine optimisations and medicine RX
- Prescribing medication
- Must be able to offer sound and professional advice in relation to clinical management issues
- Maintain and improve quality and patient safety
- Provide pharmaceutical input for clinical governance meetings
These roles will integrate with the GP surgery doctors & nurses and be part of a PCN with a great reputation.
Qualifications- A degree in Pharmacy from an accredited institution.
- Registration with the General Pharmaceutical Council (GPhC) or equivalent body.
- Strong knowledge of anatomy and physiology relevant to medication management.
- Experience in medication administration within a clinical setting, preferably in a primary care environment.
- Excellent communication skills with the ability to work collaboratively within a multidisciplinary team.
- A commitment to providing high-quality patient care and ensuring safety in medication practices
