Clinical Nurse Specialist - Derbyshire PCN

Are you an experienced palliative care nurse with a passion for innovation and improvement? Marie Curie is looking for someone highly driven and enthusiastic about Palliative care improvement, to join our NEW Primary Care Network (PCN) project.

We are seeking a skilled Clinical Nurse Specialist in palliative care to become part of our team. Your expertise will play a pivotal role in developing and enhancing this service, with a primary objective of improving how we identify patients needing palliative and end of life care by utilising advanced clinical assessments and collaborating across disciplines.

Main duties of the job

What is our PCN Project?

This project is a new and exciting opportunity to help reform and influence Derbyshire's end of life care, with a strong focus on the link between identification, deprivation and admission.

  • Our PCN Project is taking a data driven approach and focuses on four identified PCNs with low identification and high deprivation at end of life
  • North Hardwick Bolsover PCN
  • South Hardwick PCN

An exciting aspect of this project is that, with a strong focus on admission avoidance and early identification, we will be embedding Marie Curie nurses into the above GP practices networks, empowering those nurses with a 'why not home?' approach and striving to ensure the preferred place of death is an upheld choice.

Job responsibilities

We want someone who is confident and clinically proficient, with a strong focus on person centred care. Your knowledge and experience should help expand access to palliative and end of life services for individuals in underserved communities, positively affecting the well being of both patients and their carers.

This position highlights the importance of clinical expertise and strong teamwork, which are vital for Marie Curie's success. As part of the team, you will be required to comply with all applicable legal, ethical, competency, and clinical standards ensuring responsibility for your actions within your professional remit.

Key Responsibilities:

  • Deliver compassionate care and set a strong example for colleagues.
  • Lead advanced care planning with patients and families.
  • Work closely with the wider PCN team to improve patient outcomes.
  • Develop and implement best practice initiatives.
  • Coordinate patient activity and ensure high standards of care.
  • Ensure effective communication with patients, families, and the wider community.
  • Participate in clinical governance, audit, and quality improvement activities.

Job Structure: Permanent; Full Time 37.5 Hours/Week

Shifts : Day Shifts; Monday to Friday

Salary :Marie Curie Pay Scale Band 6 (Aligned with AFC) £39,959 - £48,117 Per Annum

Proposed Base: Chesterfield & Dronfield PCN

What we are looking for:

  • Experience working in Specialist Palliative Care; Proven experience in providing care to patients with complex needs, ensuring comfort and quality of life during terminal illness.
  • Experience in advanced care planning and respect discussions.
  • Registered Nurse on NMC register (Adult). Must be a registered nurse with the Nursing and Midwifery Council, ensuring adherence to professional standards and regulations.
  • Ability to connect with patients and their families on an emotional level, providing support and understanding during difficult times.
  • Demonstrated ability to contribute effectively to research, data collection, and case study development to support project delivery.
