We are seeking a highly skilled and experienced Advanced Clinical Pharmacist to join our team on a temporary basis. This role involves providing expert pharmaceutical advice to clinical teams, with a focus on cancer services. The position is available for a 3-month booking, and both full-time and part-time applicants will be considered.
Duties and Responsibilities:
Provide expert pharmaceutical advice to clinical teams, ensuring that medication regimens are aligned with the latest clinical guidelines and evidence-based practices. Ensure the safe and effective use of medications by monitoring for potential drug interactions, side effects, and contraindications to minimize risks to patient health. Collaborate with healthcare professionals, including physicians, nurses, and pharmacists, to optimize patient care by tailoring medication therapies to individual needs. Maintain accurate and comprehensive patient medication records, documenting all prescribed treatments, dosages, and any changes to therapy for ongoing care and safety monitoring.
Requirements:
- Essential:
- Up to date GPHC registration.
- Relevant Pharmacy qualification.
- Experience in a clinical pharmacy setting.
- Desirable:
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Excellent organizational and time-management skills.
