Salary: £50,952 - £57,349 per annum
Working hours: Full-time

The Pharmacy team at Pulse is seeking a highly skilled and motivated Band 8a Cross Sector Advanced Clinical Pharmacist to join our team. This role involves providing specialist clinical pharmacist support to the Medical Directorate and other wards and departments, as well as supporting GP Practices and the RDFC. The successful candidate will play a crucial role in managing chronic disease, improving medication outcomes, and ensuring patient safety across both primary and secondary care sectors.

Your main responsibilities as a Clinical Pharmacist:

You will provide specialist clinical pharmacist support to the Medical Directorate and other wards and departments. You will offer specialist clinical pharmacist support to RDFC and GP Practices, as per objectives set by the PCN Manager and Practice Manager. You will exercise professional supervision over dispensing activities for inpatients and outpatients. You will support a team of clinicians within GP Practice, taking responsibility for areas of chronic disease management. You will improve value and outcomes from medicines across both primary and secondary care sectors by managing long-term conditions, providing care and advice for patients with complex polypharmacy needs, managing patients who are taking medications commonly associated with medication errors, managing patients at risk of admission/re-admission to hospital, and ensuring patient safety. You will provide continuation of care upon discharge for the elderly, frail, and patients with complex needs. You will reduce fragmented care by developing the role and service, bridging the gap between primary and secondary care.

What we are looking for in a Clinical Pharmacist:

  • Pharmacy degree (Master/Bachelor).
  • Registration as a Pharmacist with The General Pharmaceutical Council (GPhC).
  • Postgraduate Clinical Diploma and/or equivalent experience.
  • Some Supervisory experience.
  • Prescriber qualification.
