Responsibilities

• Provide mental health triage, assessment and outcomes based intervention to meet service users' needs within the Neighbourhood Mental Health Team.
• Identify need and determine the best course of advice, support or intervention, promoting psychosocial wellbeing by supporting pathways for service users and carers.
• Ensure robust communication across mental health providers, VCSE organisations and primary care in accordance with each individual service user's needs, providing support and advice to all colleagues within the PCN as required.
• Participate in local neighbourhood Multi Disciplinary Teams (MDTs) to coordinate care and support community mental health services.
