Band 6 Community Nurse - Discharge & At-Home Rehab

Stockport NHS Foundation Trust is seeking a Registered Nurse - Band 6 to join the Discharge to Assess/Intermediate Care at Home team in Stockport. You will work in the community across 7 days, 08:00-21:00, providing care for patients on discharge and coordinating with health and social care partners.

The role offers a permanent contract, 37.5 hours per week, and a comprehensive benefits package including generous annual leave and salary progression to £39,959-£48,117 pro rata, paid yearly.
