Autonomous Community Nurse Case Manager (Adults)

Leaders In Care Ltd is recruiting a Nurse Case Manager to oversee adult complex care packages across Merseyside and West Lancashire, enabling people to live safely at home. The role offers autonomy, Monday-Friday hours and hybrid working with office and client/home visits.

You will manage assessments, care planning, and ongoing reviews while leading care teams and ensuring high standards of clinical governance. Driving licence is required.
