Responsibilities
About the Role

  • Support the service user to connect with activities, support to sustainrecovery and reach their goals set out in their aftercare plan e.g. training,employment support.

  • Facilitate post-treatment groups across the Bi Borough in community settings,outside of CGL hubs, to support individuals in progressing their recoveryjourney independently of services.

  • Provide service users with information on options available them within theservice and in their communities.

  • Collaborate with the Partnership Lead toengage partner agencies that service users can access for support followingtreatment.

  • Build positive relationships with aftercare providers to ensure recovery supportis available to support post-Rehab early abstinence i.e., Build on Belief (BOB) Fellowship Groups, Aspire2be, SMART

  • Work collaboratively with the Service User Inclusion Worker to understand posttreatment needs, ensuring the voices and preferences of service users areheard and reflected.

  • Working with Recovery Coordinators, Peer Mentors/Volunteers to provide Recovery Check-Ins


What we are looking for

  • Excellent knowledge of inpatient detox andresidential services

  • Experience of working within multidisciplinary teamsand with fostering positive relationships with partneragencies and external providers

  • Experience of working with people at all stages oftheir treatment journey and an in-depth knowledgeof the importance that groupwork and aftercaresupport offers

  • Excellent written and verbal communication skills andthe ability to provide effective advocacy whenneeded

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.


Salary Range (pro rata if part time)

CGL points 23 to 28 (£28,557.79 - £32,802.41)

Inner London Weighting (£4,236.47)
