Advanced Specialist Pharmacist - Elderly Care & Frailty

Are you a pharmacist looking for a new challenge in Elderly Care and Frailty? We're seeking an experienced clinical pharmacist to join our Elderly Care and Frailty Team. You'll help deliver a high quality clinical pharmacy service to our Elderly Care Wards and Departments.

You will work across our Elderly Care Wards and support the development of the clinical service to our Frailty Emergency Assessment Unit and Frailty Same Day Emergency Care at our Royal Stoke University Hospital site, supporting admission avoidance and de prescribing.

You'll work closely with our multidisciplinary teams and with pharmacy colleagues across both UHNM sites, benefiting from extensive training, development, research, and networking opportunities. You'll also train and support the development of junior pharmacists completing Elderly Care and Frailty rotations.

We're looking for someone who:

  • Has proven experience in delivering evidence-based, cost effective pharmaceutical care
  • Can support team development and contribute to financial and risk management
  • Is enthusiastic about improving the care of older adults
Main duties of the job
  • Utilise expert pharmaceutical knowledge to manage elderly & frail patients.
  • Utilise the independent prescriber skill daily within your scope of practice (or work towards obtaining).
  • Provide prescriber support for complex medicine regimes e.g. gentamicin, vancomycin, aminophylline infusions.
  • Support flow with efficient discharge medication supply routes in and out of hours.
  • Review adverse incident reports and educate medical and nursing teams on pharmaceutical issues to lessen risk.
  • Review and develop guidelines and PGDs; develop cost improvement initiatives.
  • Train, develop and line manage junior and trainee pharmacists.
  • Work with the Advanced Clinical Lead Pharmacist for Medicine on the strategic development of the clinical pharmacy service.

Qualifications
  • Master's degree in pharmacy (MPharm) or equivalent
  • Registered with the General Pharmaceutical Council (GPhC)
  • Postgraduate Clinical Pharmacy Diploma / MSc or equivalent Qualification
  • Independent Prescriber with appropriate annotation on GPhC Register (or willing to undertake)
  • Formal management and / or leadership qualification
Experience
  • Extensive hospital pharmacy experience
  • Comprehensive experience as a clinical pharmacist practitioner including a solid grounding in a broad range of clinical specialties.
  • Education and training experience including supervision and appraisal of junior staff and students.
  • Have recent clinical pharmacy experience in Elderly Care and Frailty.
  • Evidence of involvement in pharmacy practice research.
  • Publication of research in a peer reviewed journal / presentation at a national conference.
  • Experience of managing a team; management experience and/ or experience of undertaking appraisals.
