We are looking for a full time enthusiastic, organisedand motivated ANP to further develop our primary care team in MarketDrayton.
We are a friendly, forward thinking training practice inNorth East Shropshire providing high quality healthcare to 18,000 patients outof an impressive purpose-built modern health centre.
We already have Minor Illness Nurses, Minor Injuries Nurses,Physician Assistant, Pharmacy support, Cancer Care Coordinator and FirstContact Practitioner working as part of our overall team. We have plans for aClinical Psychologist and MH Occupational Therapist to join the team in thenear future.

Main duties of the jobTo work as an autonomous and independent clinicalpractitioner, diagnosing and delivering safe and correct clinical managementvia face to face, telephone, video and home visit consultations.
A blend of finite acute/urgent appointments and chronicdisease clinics.
Process an appropriate share of daily administrative tasksto include test results and Docman workflow.

We are a member practice of North Shropshire Primary CareNetwork (PCN), a network of GP practices covering Oswestry, Ellesmere,Whitchurch, Wem, Prees, Hodnet and Market Drayton.
Job responsibilitiesWork as part of a multi-disciplinary team to deliver safe and high quality care.
Carries out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
Delivery of care in line with NSF, NICE guidelines and evidence-based care
Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.
Where the post holder is an independent prescriber: to prescribe safe, effective and appropriate medication in line with current guidance and legislation
Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
Request diagnostic investigations and interpret within agreed protocols
To record accurate data in the patient records to an agreed standard.
The role would include a commitment to both the total triage rota, currently provided by 8 GP's and 1 ANP providing the necessary response to the new GP contract, as well as a contribution to our extended hours rota. GP supervision would be accessible during all working hours.
Person Specification Qualifications- Registered health professional (NMC/ HCPC) with or working towards MSc Advanced Practice level education
- Non-medical Prescriber
- Digital Badge holder
- Post-registration experience in autonomous clinical practice relevant to the role.
- Experience in examination of adults and children.
- Demonstrable experience of CPD and desire to actively engage in ongoing learning of self and others.
- Of working under pressure and managing varying workloads and changing demands.
- Experience of work in GP Practice & Primary Care.
- Recent experience as an autonomous practitioner diagnosing and treating patients.
