Job Description
The future is exciting. Are you ready to joinPrimary Care Sheffield (PCS) as one of our Advanced Nurse Practitioner growingcollaboration within health and social care in Sheffield provides PCS with a widerange of opportunities which can improve patient pathways and reduce cost.
This post is for 14 hours 45 minutesper week.
The post holder is an experienced nurse who,acting within their professional boundaries, will provide care for the patientfrom initial history taking, clinical assessment, diagnosis, treatment, andevaluation of care. They will demonstrate safe, clinical decision-making andexpert care, including assessment and diagnostic skills, for patients within thegeneral practice.
Thepost holder will demonstrate critical thinking in the clinical decision-makingprocess. They will work collaboratively with the PSC practices to meet theneeds of patients, supporting the delivery of policy and procedures, andproviding clinical leadership as required.
Inorder to work at this level NMC requirement for advanced practice must be met. Tobe considered for this exciting role you must have: a minor illnessqualification and a Non Medical Prescribing qualification (V300)
The work to be undertaken, could be as partof a group wide triage team or within one of our seven practices.
Main duties of the job
PCS run nine practices in the City Darnall, North Darnall,Highgate, Mulberry, City, Sothall, Harrold Street, Buchanan Road, and HeeleyGreen which delivered care to 50,000 patients, approximately 7.5% ofSheffields total population. The practices are in some of the most deprivedareas of the city with four of them located in wards which rank in the mostdeprived decile nationally.

Job responsibilities Clinical Responsibilities
Comprehensivelyassess, review and evaluate the needs of patients with undifferentiatedhealthcare needs to improve their physical, psychological and mental healthwellbeing to promote self-care and reduce the need for hospitalisation. Including the needs of Children from the dateof their first birthday.
Undertakingclinical assessment and providing the necessary treatment including the use of IndependentPrescribing in line with local/national policies, procedures and guidelines.This may be face to face or via other remote communication methods (such asTelephone triage)
Requestdiagnostic tests only where clinically indicated in line with organisational,local and national guidelines and were appropriate.
Developa plan of care in conjunction with the patient, with the application ofclinical knowledge and, analysis of symptoms and data, identification of riskfactors associated with those conditions, recognition of early signs of acuteillness, involving the patients and the carers in the care plan and ensuring itis understood, documenting safety netting plans and processes
Toutilise effective communication to aid onward referrals to specialists asappropriate. Facilitating a coordinated primary healthcare response to service usersneeds
Towork in an autonomous manner without direct supervision whilst recognisinglimitation of the practitioner role
Tocarry out enhanced clinical skills as required.
Tokeep accurate and immediate records of all interventions fully utilisingsoftware systems in place (namely System one and C- The Signs).
Toencourage clients to carry out self-care as appropriate through effectivecommunication of health care using written information as appropriate
Tomaintain patient confidentiality as per Primary Care Sheffields Policies.

Towork within the NMC Code of Professional Conduct or other relevant Codes ofConduct for Advance Nurse Practitioners.

Toprovide clinical support and mentorship to others including Nurses, Nurse associates,HCAs to meet the requirements of their own personal/professional developmentplans. This may include, where appropriate assisting with the mentoring nursesthrough work-based learning programmes.
Tosupport/facilitate the provision of Clinical Supervision in accordance withorganisational policy.
Continuous Personal Development
Tomaintain current professional registration and to ensure that all personalresponsibilities in relation to this and to the revalidation process are met,including maintenance of a professional portfolio, evidence of practice hours,professional development planning and reflective writing.
Tomeet the organisational requirements for mandatory training appropriate to therole.
Torecognise own professional limitations, practising accordingly, seeking adviceand support as appropriate.
Totake an active role in the organisational appraisal process and develop anappropriate and relevant personal development plan and work-based objectives.

The post holderwill have regular 1:1s with line mangers to support ongoing development andreview performance.

Work flexibly in a wide range of settingincluding newly developing services.
