Key responsibilities:

• Clinical care
• Care co coordination
• Collaborate closely with GPs, nurse practitioners, nurses, social workers and allied health workers
• Communicate assessment findings and care plans clearly to the primary care team
• Support care transitions following hospital discharge or emergency visits
• Facilitate referrals to community services and resources as needed
• Documentation and Quality
• Accurately document assessments, interventions and outcomes in the electronic medical record
• Participate in quality improvement initiative and projects
• Follow clinical protocols, infection control and patient safety guidelines
• Patient engagement
• Build trusting relationships with patients, carers and families in the care home setting
• Provide culturally sensitive, trauma informed and patient centred care
• Support advance care planning and goals of care discussions as appropriate
• Professional practice
• Practice evidence based medicine and adhere to clinical guidance
• Maintain current licensure, certification and continuing education requirements
• Participate in team meetings, case conferences and ongoing training
